Thank you for your interest in creating an account with Airfactz Background Screening and Reporting. As you may be aware, Airfactz is a Consumer Reporting Agency (CRA) and, as a result, we are regulated by the Fair Credit Reporting Act (FCRA) and the credit bureaus. As a result, there are some minimum requirements that must be met to create an account with Airfactz. These requirements differ by type of business. The checklist that follows lays out what must be returned for each type of business in order to create an account with Airfactz. Please return all necessary paperwork and documentation for your type of business to avoid unnecessary delays in processing your application. All forms can be found below.
REQUIRED FOR ALL TYPES OF BUSINESS
Membership Application (One page document): Must be completed and signed.
Access Security Requirements (Three page document): Must be completed and signed.
If the primary phone number listed on your application is unlisted or is a cell phone number, a copy of your phone bill must be submitted with the membership paperwork. The phone bill must be in your name or company name and the phone number must be listed on the phone bill.
REQUIRED FOR EMPLOYMENT SCREENING
End User Certification of Use for Employment Insight Reports (One page document): Must be completed and signed.
Employment Screening Service Agreement (Two page document): Must be completed and signed.
Additional Branches (One page document): If applicable, must be completed and signed.
A copy of your business license or a copy of state/federal tax records.
REQUIRED FOR TENANT SCREENING
Credit Scoring Service Agreement (Three page document): Must be completed and signed.
Tenant Screening Service Agreement (Two page document): Must be completed and signed.
REQUIRED FOR APARTMENT COMPLEX/MANAGEMENT COMPANY/REAL ESTATE AGENTS
Rental Property Information: Property Management (One page document) or equivalent document: If you would like to use your own format for the property information sheet the document must be signed and must contain the following information for each property listed: a) Complex Name, b) Address, c) Contact Name, d) Contact Phone.
One of the following: a) Three signed rental applications from past or current tenants, b) Filing in Landlord/Tenant Court, or c) Proof of membership in a rental association.
A copy of your business license or a copy of state/federal tax records.
REQUIRED FOR INDIVIDUAL LANDLORDS (Individual landlords are not eligible for Equifax products)
Rental Property Information: Individual Landlords (One page document): Must be completed and signed.
A signed rental application or rental agreement for each property listed on the Rental Property Information document.
A copy of your driver's license or government issued identification.
REQUIRED FOR REAL ESTATE AGENTS
A copy of your broker's license.
Documentation of your association with a real estate firm (i.e. copy of your contract with the firm).
REQUIRED FOR THOSE LOCATED IN
End User Certification of Compliance (One page document): Must be completed and signed.
If an individual landlord, one of the following for each property listed on the Rental Property Information document: a) copy of your filed title with proof of filing or b) property insurance documents.
REQUIRED FOR ALL TYPES OF BUSINESS RECEIVING CREDIT REPORTS*
A physical inspection of your office must be completed by an approved third party vendor. There is a $75 inspection fee. See Requirements for the On-Site Inspection for details.
*If you wish to receive our Decision Only Product, this requirement can be waived. This option is not available with Equifax products.
